Waiver of Liability | Nova Royals FC

I, the undersigned player, acknowledge, agree, and understand that:

1. Voluntarily and of my own free will, elect to participate in the NOVA ROYALS FC. I acknowledge that participation in the sport of soccer is dangerous to me and to other players and may result in serious injury or death. Furthermore, I agree that I am in good health and proper physical condition.

2. I acknowledge and fully understand that there are certain risks and hazards involved in participating in the various practices/games/tournaments/leagues that NOVA ROYALS FC associates with which may result in injury or death to me or other players including, but not limited to those hazards associated with, playing conditions, equipment, and other participants.

3. I understand that the very nature of the sport of soccer is hazardous and risky, including, but not limited to, the acts of running, jumping, stretching, sliding, diving, kicking and collisions with other players and with stationary objects, all of which can cause serious injury or death to me and to other players.

4. I willingly agree to comply with the stated and customary terms and conditions for participation in the NOVA ROYALS FC.

5. I understand that NOVA ROYALS FC does not maintain liability insurance for accident, personal injury, or other insurance, which would protect me in the event of any accident, death or injury occurring to me during or in connection with the activities, but that such insurance is the responsibility of the undersigned player.

6. I agree that before participating in any activity associated with the club, I will inspect the facilities and equipment to be used, and if I believe anything to be unsafe, I will immediately advise a representative of the NOVA ROYALS FC of such condition(s) and refuse to participate.

Further, I, the undersigned player, agree that in consideration for the right to play for NOVA ROYALS FC and in consideration for permission to play:

1. Voluntarily elect to accept and assume all risks of injury incurred or suffered by me, whether arising from the negligence of the NOVA ROYALS FC or otherwise, while practicing or playing as a member of the league, while serving in a non-playing capacity as a league member during practice or play by other players in the league, and while on or upon the premises of any and all of the activities arranged for by my team or league for practice or play.

2. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, discharge and agree not to sue the club, NOVA ROYALS FC or their owners, officers, agents, servants, associations, employees, affiliates, or any person or entity connected with the NOVA ROYALS FC for any claim, damages, costs or cause of action which I have or may in the future have as a result of injuries or damages sustained or incurred by me from whatever cause including but not limited to the negligence, breach of contract or wrongful conduct of the parties hereby released.

This form must be completed for each soccer player (participant) and must be signed by the player or player’s parent or legal guardian (if the player is under the age of 18 years old). No player will be allowed to participate in NOVA ROYALS FC practices or activities like league games or tournament games without thisform, properly executed, and on file.

For those individuals under the age (18) years (minor): As the parent and natural guardian or legal guardian of the participant, I hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the participant (player/minor) named above. I hereby bind myself, the minor, and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that I have the legal capacity and the authority to act for, and on behalf of, the minor in the execution of this club Waiver of Liability and Release form.
